Overview

Opened in 1973, ArchCare at Ferncliff Nursing Home is located on 36 beautiful acres of the former Astor family estate just outside the historic village of Rhinebeck. Our goal is to provide the best possible experience through holistic care that addresses not only the residents' physical needs, but their social, psychological, emotional and spiritual needs as well.

Under supervision of physical therapist, administer physical therapy treatments under supervision of the physical therapist and in accordance with physician's referrals and orders. ***** This position organizes and participates in rehabilitative programs that improve mobility, relieve pain, increase strength and improve or correct disabling conditions resulting from disease or injury. Therapists are expected to be highly motivated, dedicated, compassionate professionals to all our clients customers.

Responsibilities

* 
Assist in implementation of physical Therapy program and administers prescribed functional physical therapy treatment's under supervision of the physical therapist and in accordance with physicians referrals and orders.
* 
Confers with physical therapist resident and appropriate professional personnel for integration of total treatment plan.
* 
Provides instruction for personnel in techniques for stimulating and sustaining for integration of total treatment plan.
* 
Report observations of residents response to program treatment status and/or revision of treatment objectives to the physical therapist and to the physician in written progress notes which are countersigned b the physical therapist.
* 
Participates in facility in-service education programs.
* 
Supervises physical therapy assistant students.
* 
Performs other generally related duties as requested and unrelated duties in emergencies.
* 
Follows duties assigned to the position in the Home's Fire Safety Program.
* 
Provide Five Star Customer Service to all internal and external customers 100% of the time.
* 
Contributes to the overall performance in attaining the four following results: High quality care, Provider of choice, Employer of choice, Financial success.
* 
Works under the supervision of a licensed Physical Therapist. Provides treatment as outlined by the registered therapist, and in accordance with the standard of practice of Physical Therapy.
* 
Complies with the Physical Therapy Association's Code of Ethics.
* 
Dispenses adaptive/assistive equipment as appropriate, with inservice to Nursing staff or caregiver as needed.
* 
Completes documentation of patient progress using daily notes, weekly notes, care plans, nursing inservice, d/c notes, monthly statistics and other forms as required and/or assigned.
* 
Within the scope of their knowledge shows a willingness to work with new staff by answering clinical and/or procedural questions.
* 
Participates in resident and family care conferences, restraint rounds, inservices, staff meetings and other meetings as appropriate.
* 
Provides inservices to residents, family, ancillary staff and other caregivers in procedures such as range of motion, therapeutic exercise, transfers, positioning, orthotic/prosthetic care, and ambulation techniques.
* 
Assists in care and maintenance of department equipment. Reports the need for adjustment, repair or replacement to supervisor.
* 
Initiates and maintains ongoing and continuous communication with primary therapist and other members of the interdisciplinary team regarding the residents/patients/clients assigned.
* 
Maintains an orderly and neat therapy area and complies with the Center's and department's safety and infection control procedures.
* 
Attends at least one approved continuing education courses per year. (Attendance will not be mandatory until such time that the Assistant is eligible for reimbursement from the 1199 Training and Upgrading Fund.)
* 
Provides at least one department inservice per year.
* 
Other responsibilities as assigned.

Qualifications

Education Requirements:

AAS in physical Therapist assistant program.

Current N.Y.S. Certification PTA.


Experience Requirements:

Zero to six months on the job training.
