Responsibilities

Responsibilities: 

  • Coordinate and review referrals received from Liaisons, Account Executives and through the Wellsky Portal; upload to the home care EMR.
  • Enter referrals into the EMR, completing all aspects including general demographic data, physician diagnosis, and referral source data to complete the referral.
  • Perform eligibility checks and initial authorization from Managed Care. Document co-pay, co-insurance, and deductibles for patient consent.
  • Communicate with the home care team, referral source, potential patients, and their families.

Qualifications

Qualifications: 

  • 1 year experience

Education: 

  • High School Diploma or equivalent

Salary

Salary: $54,500 - $56,500 per year
