Responsibilities: 

• Complete Admission, Re-Admission, Annual, and Significant Change MDS Care Area Assessments (CAAs), ensuring compliance with state and federal guidelines for resident-centered care plans.
• Develop and implement comprehensive care plans, including measurable objectives, time frames, and services needed to support residents’ physical, mental, and psychosocial well-being.
• Prepare and communicate Baseline Care Plans within 48 hours of admission, detailing initial goals, care instructions, and medications.
• Lead care plan meetings, coordinating with the interdisciplinary team (IDT) to ensure individualized care and involving residents, families, and representatives in the process.
• Maintain up-to-date care plans reflecting current clinical data, ADLs, and advance directives; collaborate with Nurse Managers and attend daily and weekly care meetings.
• Conduct quality assurance audits, daily clinical rounds, and actively engage with residents and families to uphold high standards of customer service and quality care.

Job Type: Full Time

Salary: $48.00 per hour

Education:

• High School Diploma or equivalent. College degree preferred.

Experience Requirements

• Minimum 1 year of MDS experience preferred.

License Requirements

• Current NYS RN license.
• MDS Certification preferred.
