Overview

The Quality Relationship Representative provides support to the Provider Relations Department in the oversight of Social Adult Day Centers (SADC) and Adult Day Health Centers (ADHC). Responsible for conducting annual site visits to contracted centers for multiple lines of business to ensure compliance with regulatory requirements.  Completes all audit related activity including inspection, documents findings and submits required reports.  Serves as liaison between Provider Relations and Quality Department. Assist in collection and processing of credentialing material when needed.  Assists in the investigation and response of complaints regarding day center providers and adheres to required response time as outlined in policy and procedures.  Participate in any/all training and educational activities necessary to fulfill at least the minimum requirements as specified within your departmental goals.  Provides support in all aspects of Quality improvement activities including but not limited to 5 Star Rating Program, Appeals, grievances, focused studies, interventions and other QI related activities. Under supervision, prepares basic reports for review and presentation within the department and to internal committees.

Responsibilities

Provide Five Star Customer Service to all internal and external customers

Serves as liaison between Provider Relations and Quality Department

Assists in the investigation and response of complaints regarding day center providers and adheres to required response time as outlined in policy and procedures

Assist with the implementation and monitoring of interventions for continuous quality improvement.

Assist with day-to-day quality improvement activities including but not limited to collecting data, entering data, conducting monthly internal audits, monitoring progress of assigned QI projects, and preparation of basic reports for presentation at assigned internal committees and subcommittees.

Assist with internal and external audit preparation

Qualifications

Excellent oral, written and interpersonal communication skills.  Computer proficiency with a developed knowledge of Microsoft applications (Word, Excel, & PowerPoint).  Ability to interact with all levels of staff in a diplomatic manner. Exceptional customer service skills. Ability to multi task and comfortable with deadlines. Good prioritization and time-management skills.  Ability to work independently.
