Overview

ArchCare Senior Life is a Program of All-inclusive Care for the Elderly (PACE). PACE is a community-based healthcare program created for people 55+ who require nursing-home-level care, but prefer to receive it in their own familiar surroundings.  ArchCare Senior Life is both the provider and payer for those enrolled into our program, and serves a multi-ethnic, multi-cultural and multi-linguistic community of New Yorkers. 

 

ArchCare Senior Life PACE is seeking an Occupational Therapist to join our team.    You will join our vibrant Interdisciplinary Team (IDT) model, collaborating with Medical, Nursing, Physical Therapy, Speech-Language Pathology, Recreation Therapy, Dietary Services, Home Health Aides, Transportation & more!  The Occupational Therapy Coordinator is responsible for evaluation and treatment of patients, but also has OT-responsibilities related to the insurance/payer side of our business.  Time is divided between our clinic location, home visits, and telehealth.  As we continue to progress in our post-COVID recovery, some remote work time is anticipated. 

 

The ideal candidate is self-directed, flexible, and has excellent time-management and communication skills.  They value collaboration, and thrive in an inclusive multi-cultural work environment.  The desire to make a lasting impact on the life of those in our care motivates them.  They have experience in the SNF &/or Home Health settings, and are excited to service similar populations in our innovative PACE model.  They are solutions-oriented, and are willing to seek out-of-the-box solutions.

Responsibilities

  • Evaluates all individuals upon their enrollment in the program, and when referred by the Primary Care Provider.
  • Provides individualized, goal-directed, evidence-based occupational therapy intervention to restore physical function, compensate for chronic impairments, and maintain functional level; provides caregiver/family training.
  • Contributes to care coordination and discharge planning of individuals who are hospitalized.
  • Actively participates in the Interdisciplinary Team (IDT) model by attending IDT meetings, contributing to the plan of care, and collaborating with the team for best health outcomes. 
  • Screens for, makes referrals to and collaborates with Physical Therapy & Speech Language Pathology.
  • Provides evidence-base falls intervention and education in group settings.
  • Uses telehealth platform for virtual therapy and check ins. 
  • Collaborates with the Rehab Department head to develop programming that best supports the individuals and the community.
  • Maintains treatment records through the electronic medical system, meeting all federal, state, and organizational regulations.
  • Meets with leaders and colleagues via phone and teleconference; contributes to the development of the Rehabilitation Department as a whole.

Additional job requirements may include:

  • Cross-covers occupational therapy at PACE sites in other regions on occasion as needed

Therapists with homecare &/or SNF experience are ideal fits.  Bilingual Spanish/English is a plus.
