Infection Control Nurse Coordinator Home Health Care

The Community Infection Control Nurse performs acts, which require substantial specialized knowledge, judgment, and nursing skills based upon principles of psychological, biological, physical, and social sciences. S/he is responsible maintaining monitoring an effective, community based infection prevention and control program, as well as assisting the head of the corporate infectious control lead in developing the program.

Essential Functions:

• Ensuring that the infection prevention and control program at ArchhCare (aka PROTECT ArchCare) is followed and maintained in the community based programs (ArchCare Advantage, ArchCare AtHome, ArchCare Community Life, ArchCare Senior Life).
• Assists in developing and implementing program initiative, policies, and education related to infection prevention for associates and patients.
• Monitor patterns, prevalence and trends, conduct surveillance and evaluate healthcare associated infections using epidemiologic principles by review and interpretation of microbiology, laboratory and diagnostic reports and medical records in the community programs. Monitors the effectiveness of the PROTECT ArchCare program in the community programs.
• Supports teamwork essential for compliance with accrediting and regulatory agency standards through surveillance, effective prevention and control activities specific to a variety of practice settings, and accurate reports.
• Develop for the community programs an education and training program on infection prevention and control, in conjunction with the infection and prevention medical director, the corporate infection and prevention and controll nurse and ArchCare’s education and training department.
• Participate in said educational and training sessions as indicated.
• Monitors and evaluates prevalence of healthcare associated infections in the community programs.
• Activities may include:
  • Application in the community programs of the nursing process in assessing, planning, and evaluating the PROTECT ArchCare program.
  • Communicate with appropriate sources in response to clusters, outbreaks as well as emergency situations.
  • Participates in the PROTECT ArchCare committee.
  • Perform Infection Control Risk Assessments in the community programs as necessary and monitor.
  • Participate in facility-wide Emergency Preparedness activities related to infection control measures relating to communicable disease outbreaks.
  • Other duties as assigned.

Formal Education

Bachelor's Degree, Graduate of an accredited School of Nursing.

Experience

Experience: Unless otherwise indicated, a minimum of five years of current experience and ideally, within the last three years, in a comparable job classification

License, Registration, and / or Certification Requirement

Licensed to practice as a Registered Nurse in New York State

Certificate in Infection Prevention and d Control (CIC)

Minimum Knowledge

Ability to work effectively within role independently and with other team members.

 Ability to organize and complete work in a timely manner.

 Ability to read, write and effectively communicate in English.
