    • Delegates responsibility for completing section of the MDS.
    • Provides staff with schedules of start and completion dates of MDS assessments and Comprehensive Care Plan (CCP) meetings.
    • Reviews MDS assessments, signs and dates form to certify completion of assessment by all disciplines.
    • Instructs staff on how to integrate MDS information into existing Resident Assessment and Care Planning Practices.
    • Oversees proper implementation of Trigger Legends and Resident Assessment Protocol Summary (RAPS) as an integral part of the care planning process.
    • Maintains all actual records for seven (7) years and retrieves records upon request of Sate or Federal representatives.
    • Completes Patient Review Instrument (PRI) documentation for all new admissions that are not from a NYS health facility.
    • Identifies Medicare eligibility and coverage status and informs Fiscal Services of findings.
    • Informs all disciplines of Medicare eligibility and coverage by posting MDS Assessment schedules for SNF PPS process.
    • Notifies Fiscal/Billing Services when Medicare Part A coverage is discontinued.
    • Notifies Resident or responsible party when Medicare Part A is discontinued, and issues notification letter.
    • Provides Fiscal Services with required Medicare documentation for billing process and demand billing.
    • Provides necessary documentation for Medicaid applications when requested by Financial Screener.
    • Checks census daily and keeps discharge and re-entry tracking forms current.
    • Competes QA/PI audits as required, ie: Triple check for Part A, State Survey Corrective Action
    • Utilizes software to ensure accuracy, ie: Pointright and Care Analytics.
    • Assists with residents when needed.
    • Performs other duties as assigned by the Vice President.


    • Graduate of an accredited school of nursing; BSN preferred
    • Five years of experience within the field of geriatric nursing
    • Three years of MDS experience
    • Current NYS Nursing License
    • Certification as a PRI Assessor
    • Resident Assessment Certification; within one year of employment
