  • Overview

    The Community Outreach Worker position outreaches to the community assist in case finding for programs, provides services in the Walk-In program with entitlements, medical assessments, and other assessments; assist with caseloads, will be culturally and linguistically competent and assist with the needs of other South Bronx Grant and Charitable Programs.


    1. Carries out the agency’s mission philosophy, goals, and objectives within guidelines of agency policy and position function.
    1. Respect and maintain patient/employee confidentiality.

    2. Interprets and implements the agency’s philosophy to staff and members of the community.

    3. Establishes and maintains effective working relationships with patients, families, agency staff and other community agencies.
    1. Gathers pertinent medical and non-medical information from patient/family/staff or from patient record on referred patients.
    1. To assist staff in understanding cultural patterns and behavior.
    1. To maintain accurate records and reports.
    1. To provide family centered care and hold a number of cases from programs; ensuring that all documentation are complete (i.e. plan of care, progress notes, intakes, medical assessments, etc)
    1. To accept and utilize supervision in performing duties.
    1. To participate in evaluating effectiveness of his/her service and assess overall job performance.
    1. To engage in community outreach activities which include, but are not limited to: office based street walk-in; home and community based outreach to individuals and families; etc.
    1. To asses and provide appropriate in-house and community based referrals.
    1. To provide comprehensive culturally competent, bilingual, HIV/AIDS Education.
    1. To provide and asses for risk and need of resources, such as HIV Risk Assessment, Walk-In Progress Note Assessment, Ryan White and Health Home Assessments, etc.
    2. To encourage HIV and HCV testing when indicated, provide pretest and posttest counseling, and referrals to appropriate testing sites.
    1. To encourage and facilitate compliance and access to follow up appointments.
    1. To assist in all other aspects of program services, program development, etc.


    • An Associate’s Degree in Social Services and/or allied field
    • 2 years' experience
    • Spanish-English bilingual
