  • Overview

    The Long Term Home Coordinator assists the supervisor in the coordination and management of home health care services in the Long Term Home Health Care Program (LTHHCP) in compliance with applicable laws, regulations and Agency policies.

    Responsibilities


    • Educate all clinical staff who conduct home visits on how to continue the conversation with the patient about future care needs, continuity of care within the health system.

    • Maintain and inventory of collateral materials for ArchCare programs and distribute to clinical staff and ensure material is disseminated to those we serve.

    • Track admissions to the CHHA that can benefit from on-going support and maintain contact with them, begin to disseminate collateral material and seek permission to have representatives of the right supportive program to contact them about potential enrollment.

    • Track conversion rates of referrals made by Outcome’s manager.

    • Collaborate with ArchCare programs that referrals are made to ensuring proper communication with the patient and a smooth transition to the new program.

    Qualifications

    Education Requirements



    • Bachelor’s degree in marketing, social work or community service preferred


    Skill Requirements



    • Proficient in Office Suite, Word, PowerPoint, Excel, etc.

    • Good communicator, comfortable presenting and speaking in front of large groups.

    • Effective and comfortable providing education on various topics.
