Responsible for providing administrative and social services support to TCC Social Service and Case Management Teams including Social Service Director, Case Management Director, Social Workers and Case Managers. Ensures all administrative duties, including data entry, reports and community applications are completed timely. Acts as a liaison for the social service department with community supports such as Social Security, HASA, Housing entities and Community Resources. Works under general direction.
Works with TCC staff to provide administrative and social service support in the develop and processing of reports, spread sheets and necessary community applications
Answers and responds to calls to social service general phone line
Collaborate with social services and care management to manage and monitor the KIDs portal
Monitors and organizes QAIP reports monthly and as needed
Completes necessary community applications for social security, housing and other community resources
Assist the interdisciplinary team with inviting resident/ families to Comprehensive Care Planning Meetings- Monthly by sending invitations
Assist the social service department with Assessments, MDS and established Care Plan meetings as needed.
Covers assigned units as needed to provided social service support
Acts as a resource to all social work and case management staff as needed
Education: Bachelor’s degree in social work, social services, human services.
