LPN Intake CSR

The Intake LPN serves as contact between the agency and the community and referral sources to manage and process Intake referrals with the oversight of an RN for clinical related information as needed; to perform data entry, and general office tasks associated with Central Intake Department support.

• Work as a team member with external referral sources, other Customer Service Representatives, Intake Supervisor and other departments by screening incoming phone calls, faxes and electronic communications, assuring that communications are directed appropriately and initiating the collection and data entry of demographic patient referral information.
• Process referrals, including determining the appropriateness of a patient for homecare and data entering referral information in a timely manner.
• Maintain patient records in accordance with established protocols and policies.
• Provide support for regulatory initiatives.
• Communicate with area hospitals, physicians, & facilities to insure accuracy and completeness of referral information.
• Perform other functions and tasks to promote the smooth operation of Intake Department and coordination of operations with other departments.
• Perform, when needed, the verification of third-party insurance and determines if coverage is sufficient to meet the patient’s needs at home.

• 2-3 years of broad office experience. Basic computer knowledge, data entry experience required, and working knowledge of medical terminology and healthcare insurance authorization processing preferred. Experience in dealing with internal and external customers in a fast-paced multi-task environment required.
• High School Diploma required. Graduation from a two-year college with an Associate’s Degree preferred. Medical terminology preferred.
• Ability to work in fast paced environment and strong organizational skills
• Understanding of medical terminology and ICD-10 coding along with knowledge of home care regulations, requirements and Conditions of Participation.
