Overview

The Clinical Manager is responsible to direct the clinical operations of assigned care teams to ensure the provision of appropriate care and services to members. Ensures that members have initial and periodic re-assessment for individualized care planning. Develops staffs that have personal accountability at the Care Team level for quality care and member satisfaction.

Responsibilities

  • Provide direct supervision and management support to all Care Team staff members to ensure the evidenced based decisions with critical thinking skills are completed for each member’s condition and changes in that condition
  • Ensures documentation of issues is complete and maintained in the member’s health record
  • Models effective clinical skills, appropriate critical thinking skills and appropriate care decisions to assist staff to develop their own competency
  • Provide effective systems and protocols to ensure compliance with care management policies for assigned teams to enhance coordination with other service providers
  • Coordinates the daily Team meetings to assure that member issues are addressed timely.
  • Ensure that process flow is accomplished for paperwork and/or data entry is completed in a timely manner
  • Ensure that appropriate advance preparation is completed to support efficient work flow practices
  • Manage day to day problems related to member care and service delivery by providers for assigned teams
  • Ensure effective management of personnel and material resources to include performance appraisals are completed, meeting regulatory requirements related to orientation, in-services, and performance improvement for assigned teams
  • Oversee compliance with Care Planning for assigned teams
  • Participate in the QAPI Program to determine opportunities for improvement and support for continued PI Projects
  • Participate in the QAPI Program to determine opportunities for improvement and support for continued PI Projects Provide Administrative on call coverage on a rotating basis.
  • Ensure coordination with other suppliers and vendors
  • Performs other related duties, as required

Qualifications

  • Two (2) years of experience in management in a community based program caring for older adults at a supervisor level or higher
  • Experience in a MLTC, CHHA, or LTHHCP is highly desirable
  • Demonstrated skills in supervising, organizing and directing the work of others required
  • Bachelor's Degree 
  • Masters Degree in Nursing, Health Care Administration, Public Administration, Business Administration preferred
