  • Overview

    This position is responsible for the re-evaluation of members every six months or for change in condition, focusing on changes in functional status and in home supports to member.

    Makes appointments with members in advance of reassessment date.
    Conducts semi annual and change of condition re-assessment to determine clinical eligibility.
    Conducts a clinical assessment focusing on member's change in functional status.
    Conducts an assessment of the home supports available to the member.
    Completes SAAM Tool/UAS tool and 180 day reassessment note.
    Develops an initial service plan for the member and secures their agreement with the new plan.
    Discusses the findings from the assessment with the Care Team during reassessment meeting
    Revises the Home Health Aide Plan of Care and leaves a copy in the home.
    Performs other related duties as required.


    Education & Experience
    Formal Education: Bachelor's Degree
    Experience: 1 year
    License, Registration, and / or Certification Requirement: Yes
    Minimum Knowledge: Requires advanced knowledge of a specialized or technical field or a thorough knowledge of the practices and techniques of a professional field.
    Language Ability: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.


    Education Desired
    Bachelor's degree preferred.


    Experience Requirements
    Experience in home care, geriatric care, discharge planning, case management or long term home care.
    One year of experience in community based program caring for older adults.
    Demonstrated skills in planning, organizing and directing the work of others required.
    Experience in intake or an admission team of MLTC, CHHA, or LTHHCP is highly desirable.


    License Requirements
    Registered Nurse currently licensed in New York State.
