Overview

The Care Manager Social Worker plans and coordinates care management for members. Works in collaboration with Care Manager Registered Nurse and the Clinical Services Associate to collaborate on the plan of care for the member, and also monitor effectiveness of the care plan and promote member satisfaction with the service provision.

 

Responsibilities

* 
Works collaboratively with team to ensure safety and well being of members.
* 
Communicates with enrollment personnel to ensure a smooth transition at the start of care.
* 
Maintains regular on-going telephonic communication with members and care givers.
* 
Maintains regular on-going direct social work services to members and caregivers.
* 
Identifies member strengths, deficits, and available resources and services in order to enhance member’s quality of life.
* 
Educates members and family in health management and community resources.
* 
Develops treatment plan, evaluates member progress regularly and modifies treatment plans when necessary.
* 
Communicates regularly with the member’s Primary Care Physician (PCP)
* 
Participates and assists as necessary in development of the reassessment Plan of Care and makes changes in the plan of care based upon findings.
* 
Provides direct services, such as counseling, psychotherapy and case/care management.
* 
Maintains oversight for the member’s care management during long term SNF placements and short term sub acute placement for rehabilitation.
* 
Ensures effective discharge planning for transfer to the community and resumption of services.
* 
Documents any change in member condition and authorizes services to visit member for an assessment and possible change in the care plan.
* 
Participates in prevention, surveillance and control of infection via infection control policies
* 
Intervenes as necessary to prevent dissatisfaction and resolve concerns and/or grievances.
* 
Participates in QAPI initiatives within ArchCare Community Life
* 
Performs other related duties, as required

 

 

Qualifications

Performance Management

Formal Education: Master's Degree
Experience: 1 year
License, Registration, and / or Certification Requirement: Yes
Minimum Knowledge: Requires advanced knowledge of a specialized or technical field or a thorough knowledge of the practices and techniques of a professional field.
Language Ability: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.

Education Requirements
*
Masters Degree in Social Work.

Experience Requirements
*
Experience in working in a case/care management setting, multi-cultural setting, experience in community health practices, experience with adults with chronic disorders, the frail elderly and the developmentally disabled.
*
At least one (1) year of experience in a community based program caring for older adults required.
*
Demonstrated skills in planning, organizing and directing the work of others required.
*
Ability to work effectively with adults with chronic disorders, the frail elderly, and the developmentally disabled.
*
Strong assessment skills and cultural competency required
