The Medical Care Coordinator is responsible for the provision of professional medical care coordination services to Health Home patients and their families. All servives are provided in compliance with applicable laws, regulations, and Agency policies.


  1. Carries out the agency’s mission philosophy, goals, and objectives within guidelines of agency policy and position function.
  2. Interprets and implements the agency’s philosophy to staff and members of the community.
  3. Establishes and maintains effective working relationships with patients, families, agency staff and other community agencies.


  1. Gathers pertinent medical and non-medical information from patient/family/staff or from patient record on referred patients.


  1. Assesses the environment of the home and community to identify elements which impact negatively on the patient’s/family’s ability to function.


  1. Develops a plan of care in collaboration with the patient, nurse, social worker, and other health professionals.


  1. Provides concrete medical care services to enable patients to receive services.


  1. Provide direct medical care coordination services to patient/family or makes appropriate community/health referrals.


  1. Conducts intake interviews with clients and/or their affected family members who request care coordination services and maintains up-to-date clinical notes on clients.


  1. Delivers medication adherence, partner notification, co-morbidity prevention, and clinical trials participation behavioral interventions to clients.


  1. Consults with supervisors and staff as needed and performs HIV C&T as needed.


  1. Attends training seminars, staff meetings and supervision on a weekly basis and in a prompt manner.
  2. Helps patients and families to develop capabilities that permit them to make optimum use of health and social facilities.


  1. Participates in conferencing and planning with other disciplines caring for the patient.


  1. Transfers cases to the appropriate social agency if the need for continued service exists.


  1. Prepares progress notes, documents in patient’s record, prepares plan of care, prepares coordination, etc.
  2. Performs all other related duties, as required.


  1. Participates in performance improvement activities.


  1. Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.


  • Bachelor’s in Social Work, Human Services, Sociology and/or related field from an accredited school
  • Experience in health care/social work setting as a member of a multidisciplinary team.
  • Bilingual required (Spanish/English)
