  • Overview

    Performs all screening assessments for newly admitted, re-admitted and hospital returned patients residents, in the areas of speech, language, swallowing, hearing and general communication. Evaluate patients residents according to the principles and practices of speech therapy. Collaborate with interdisciplinary treatment team. Interprets treatment rationale to the patient resident and family and maintain communication with the family when needed. Participate in patient resident care services as directed by supervisor in accordance with in-service training. Perform therapy services in accordance with current applicable federal, state, and local standards, guidelines and regulations (as may be directed by the administrator) to assure that the highest degree of quality patient resident care can be maintained at all times.



    Screens patients/residents promptly as scheduled per departmental protocol and policy. Makes appropriate recommendations for interventions arising from screening assessments.

    Evaluates patients/residents under a physician’s order.

    Develops or modifies treatment plans that reflect treatment strategy, purpose and anticipated outcomes based on the initial evaluation or re-evaluation.

    Provides direct participant treatment in accordance with an established plan of care under a physician’s order.

    Provides and or suggests specialized adaptive equipment to improve participant’s ability to communicate or swallow.

    Involves the patient/resident and or family in planning objectives and goals; including participation in the planning and scheduling of therapy sessions.

    Communicates effectively with patients/residents.

    Communicates effectively with families/designated representatives and or caregivers.

    Consistently communicates patient/resident related matters to Rehabilitation Team.

    Coordinates speech therapy schedule with other departments.

    Interacts and practices in collaboration with a variety of professionals (physicians, nurses, educators, social workers, occupational therapists, etc.)

    Assists in transporting patient/resident to and from speech therapy treatment areas.

    Consistently completes documentation in a timely manner (progress notes, interim notes).

    Establishes the discharge plan and documentation of discharge summary/status.

    Completes efficiency and productivity records daily.

    Documents progress and conducts periodic re-evaluations promptly and accurately according to the department’s documentation system.

    Completes additional documentation as required (treatment grids, equipment lists, care plans/speech therapy.

    Supervises departmental consultants, interns and support personnel; pertaining to the operation of speech therapy

    Performs additional administrative duties, including but not limited to: assigns and oversees patient/resident care treatment, execution of physicians orders, review of patients’/residents’ medical records, monitors current inventory of items used, informs supervisors of need to replenish.

    Participates in clinical program development and implementation.

    Recognizes patients’/residents’ right to refuse treatment; honors the refusal and reports each instance to the Director of Rehabilitation.

    Investigates the cause of the refusal and implements appropriate modifications as indicated.

    Ensures that patient/resident rights to fair and equitable treatment, self determination, individuality, property and civil rights including the right to wage a complaint are well established and maintained at all times.

    Review complaints and grievances made by the patient/resident and or designated representative; reports grievance to the Director of Rehabilitation, indicating what action(s) were taken to resolve the complaint.

    Provides positive customer relations at all times.

    Maintains the confidentiality of all participants care information.

    Demonstrates ability for continuous learning and advancement in speech therapy practice techniques.

    Attends continuing education courses relevant to current practice.



    Formal Education: Master's Degree
    Experience1 year - In Geriatric Setting
    License, Registration, and / or Certification Requirement: Yes
    Minimum Knowledge: Requires advanced knowledge of a specialized or technical field or a thorough knowledge of the practices and techniques of a professional field.
    Language Ability: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.  Bilingual Spanish (Fluent) preferred due to the population being serviced.


    Education Requirements
    Education normally acquired with a Masters Degree and graduation from an accredited school in Speech/Language Pathology.


    License Requirements
    Licensed by New York State as a Speech/Language Pathologist.
    Required licenses, registrations and certificates.
