Overview

Responsible for providing comprehensive assessment and coordination of 24 hour care for PACE participants as part of the PACE interdisciplinary team. Plans, organizes and conducts a comprehensive program of individual and group services to enable participants to achieve optimal levels of independent functioning and improve or restore functions which have been impaired, reduced or permanently lost due to illness or injury. Works under general supervision. Supervises physical therapy, occupational therapy and speech therapy when rehab director is unavailable. Monitors therapists’ work and performance as well as provides constructive feedback, direction and training. Enforces policies and procedures. Assumes clinical responsibility and serves as contact person for his her assigned specialty area (Parkinson’s program, deaf population, developmentally disabled participants). Is available to administration and other staff for consultation regarding rehabilitation services and specialty programs. In conjunction with the Rehabilitation Director, responsible for the development of philosophy, program objectives, job descriptions and specific policies of the department.

Responsibilities

* 
Monitors effectiveness and outcomes regularly and keeps the team informed as to participate progress and level of independence. Remains alert to pertinent input from other team members, participants and family members/caregivers.
* 
Maintains participant records by completing all required PACE documentation and ensuring compliance to program standards and policies.
* 
Monitors participant progress and outcomes through assessment of care provided, including need for revisions to treatment plan. Coordinates with other members of the interdisciplinary team to ensure that the comprehensive plan of care is followed and revised as needed.
* 
Counsels and provides expert resource to patient/caregivers/family by providing education/consultation for related patient care issues, including referrals within and outside of ARCHCARE SENIOR LIFE as necessary.
* 
Performs and assists participants with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals.
* 
Assists participants, family caregivers and other staff with use of durable medical equipment as a component of patient care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthotics, prosthetics, and exercise/strengthening equipment.
* 
Develops and implements the rehabilitation components of the interdisciplinary team plan of care to promote restore or maintain the participant's optimum level of functioning.
*

Travels to patients' homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.

Develop and maintain a good working rapport with inter-department personnel as well as within the facility to ensure that services are properly maintained
* 
Delegate and develop work assignments and schedules
* 
Oversee the time management, work productivity, and patient care in all rehab areas.
* 
Meet with administration, medical and nursing staff as well as other related departments in planning therapy services as needed.
* 
Participate and assist in departmental studies and projects as assigned or that may become necessary

Qualifications

Formal Education: Bachelor's Degree
Experience: 3 years
License, Registration, and / or Certification Requirement: No
Minimum Knowledge: Requires ability to interpret/extract information and/or perform arithmetic functions.
Language Ability: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

 

Education Requirements
*
Bachelor's degree in Occupational Therapy from a program approved by the New York Department of education Certificate in Occupational Therapy from a college or university whose program is registered by the State education department or accredited by a national accreditation agency.

 

Education Desired
*
Master's degree in Physical Therapy, Occupational Therapy or Speech Therapy from a program approved by the New York Department of educations Graduate Certificate in Physical Therapy, Occupational Therapy or Speech Therapy from a college or university whose program is registered by the State education department or accredited by a national accreditation agency.

 

Experience Desired
*
Have 3 year of experience with a frail or elderly population
*
Current license and registration to practice as a Physical Therapist, Occupational Therapist or Speech Therapist in the State of New York.
