Overview

 Manages & coordinates organization efforts that performance management and quality improvement programs are developed and managed using a data-driven focus that sets priorities for improvements aligned to strategic imperatives.  Assures that organization-wide performance & quality improvement initiatives are focused and aligned on improving operational and program efficiencies and effectiveness.  Provides leadership for policy and procedures development.

Responsibilities

  1. Establishes a continuous performance and quality improvement effort & monitoring & reporting system. 
  2. Collects, analyzes, documents and reports the status of performance & quality improvement efforts and its impact in achieving strategic targets and benchmarks within the organization.
  3. Applies, coaches, uses and facilitates tools and methods to achieve improvement and quality outcomes and process efficiencies and effectiveness.
  4. Evidence based
  5. Collaborates, develops, revises and reviews policies and procedures for approval by the organization’s designated body.
  6. Collects and conducts analysis of data related to clinical performance that informs improvement in patient safety and the patient experience.
  7. Coordinates organization’s efforts in in “survey readiness” for evaluation of clinical performance and achievement of quality outcomes by external regulators.
  8. Participates, coordinates and facilitates in partnered quality improvement activities, e.g. DSRIP, Collaborative Initiatives, as needed.

Performs other duties, as required

Qualifications

RN (required), BSN preferred; 3 years home health care experience; 2 years experience in quality improvement or compliance preferred; CPHQ desirable
